We recognize that for many families who have one car or more than one child, the drive from the base to our office in Visalia can often be a hardship.  Our goal is to make therapy services more accessible to you by coming to your home.  In order for us to do that, each of the therapists must have a sponsor.  As a sponsor, you will need to fill out the cover page of the “Base Access Request” which is in the PDF form below.

In section 9, please request a full year pass, as this will alleviate the necessity of multiple requests for passes and the potential lapse in therapy services.  Please be sure to sign in section 11 where it says “Requester Signature”. 

In section 15, we have already filled in the required information with the exception of your home address, which each sponsor will need to do. Once you have completed the cover page of the the Base Access Request, we will complete the “Identity Proofing and Applicant Information” forms.

When all forms are completed, they will need to be turned in as one complete packet at the Visitors Office by the sponsor family. We will make arrangements to meet you at the Visitors Office to give you our completed portion of the packet.  It is a very easy process and we look forward to being a service to your family.

We also have a client intake form and office policies and procedures forms that ALL new families must fill out. Please carefully read our “cancellation and no-show” policy.

Criteria for in-home speech therapy services

1. The initial assessment must be completed in our office. If your child has had an assessment done within 6 months, then all you will need to do is forward a copy of  the initial assessment report to our office and we will be able to develop a treatment plan.

If your child has a school IEP that is 6 months old or less, we will not need to do an assessment in the office.  An assessment will need to be done if your child has never had an assessment OR if the prior assessment or school IEP is more than 6 months old. 

2. Therapy sessions will need to be conducted in a quiet/semi-quiet area of the home.  This can be at the dining table, living room, or a home office. 

We encourage parents to be a part of the therapy session if possible, however, siblings cannot be part of the therapy sessions unless it serves to benefit the child receiving the therapy.  There are occasions when children participate more in therapy when a sibling is part of the session. 

We bring our own therapy materials, but ask that you have 1-2 favorite toys or activities that will serve as reward/motivation for your child.  

3. Please notify our office as soon as possible if you are not able to keep your appointment for the day. On days when we have cancellations, we may call the remaining families to see if you are able to move your appointment ahead or back a few minutes. We do this in order to avoid too much down time while the therapists are on base. 

We do understand if you are not able to adjust your appointment time on those days we have cancellations and we will then keep your appointment as scheduled.
